Section 1 should match your Voter Registration Card. If you need to verify that you are registered, please contact our office.
Section 2 is to be completed only if you need the ballot mailed to a different address than you are registered at.
Section 3 lets you mark the elections you would like absentee ballots for in 2012.
Section 4 applies to the 2012 Primary Election only. Please read carefully and mark your choice.

Section 6 is for your signature and the date. Please don’t forget to sign this form.

The request can be returned to our office at:1230 Main Street, Lewiston, Idaho. Our mailing address is: PO Box 896, Lewiston, Idaho 83501. An absentee ballot will be mailed to you prior to the election.
