1. EVIDENCE OF CITIZENSHIP
   • CERTIFIED Birth Certificate with file date, original seal and signature from issuing office. (HOSPITAL BIRTH RECORDS OR CENSUS RECORDS WILL NOT BE ACCEPTED) or;
   • Previous U.S. Passport (preferred in place of Birth Certificate and will be returned to you) or;
   • U.S. Citizens born abroad - Consular Report of Birth, Certificate of Birth or Certificate of Citizenship or;
   • Naturalized U.S. Citizens - Certificate of Naturalization
     
     
2. PROOF OF IDENTITY
   • State Drivers License or;
   • Current U.S. Passport (Cannot be used for identity, if used for citizenship)
   • Expired forms of ID are not accepted
   • Parents ID will be used for children under 16 years old
     
     
3. PHOTOGRAPHS
   • Two recent identical passport photos. These can be obtained from AAA, Sears, Wasem's Drug or Costco
   • Portrait photos are not suitable
4. FEES
   • Age 16 and older:
     Passport Book: $75 Check or Money Order Only Payable to U.S. Department of State
     Passport Card: $20 Check or Money Order Only Payable to U.S. Department of State
     AND
     Process fee: $25 Cash, Check or Money Order to Nez Perce County
     
   • Age 15 and younger:
     Passport Book: $60 Check or Money Order Only to U.S. Department of State
     Passport Card: $10 Check or Money Order Only to U. S. Department of State
     AND
     Process fee: $25 Cash, Check or Money Order to Nez Perce County

• ALL Applications must be SIGNED IN THE PRESENCE OF AN ACCEPTANCE AGENT
• 16 years and older must sign their own application
• UNDER AGE 16 APPLICATIONS REQUIRE BOTH PARENTS' SIGNATURES
• PROCESSING TIME: Approximately 6 weeks for delivery
