Absentee Ballot Request Information
To receive an absentee ballot by mail, complete all information on the Application for Absent Elector's Ballot. This form is a pdf form and is fillable before printing. Be sure to sign and date the form and then return it to our office. This request can be used for one year if all the appropriate boxes are checked. We are located at: 1230 Main Street, Lewiston, Idaho. Our mailing address is: PO Box 896, Lewiston, Idaho 83501. An absentee ballot will be mailed to you prior to the election.
